Is Edesy Forms HIPAA compliant?

Edesy Forms does not claim HIPAA compliance and does not sign Business Associate Agreements. Do not use it to collect medical records or other sensitive health information unless you have checked that it meets the rules that apply to your practice.

What can a clinic use it for?

Everyday admin forms: new patient registration with contact details, appointment requests, callback requests, patient feedback and NPS surveys, health camp or event registrations, and staff or vendor forms.

Can patients book a specific time slot?

Not automatically. There is no calendar sync or slot booking. Patients can request a preferred date and time with a Date question, and your front desk confirms the appointment.

Can I restrict who fills in a form?

Yes. You can password-protect a form, limit it to one response per browser, cap the total number of responses and set dates when it opens and closes.

Can patients upload documents?

Yes, with a File upload question. Files can be up to 10 MB each on the Free plan and up to 100 MB each on Pro. Only ask for documents you actually need.

Can patients sign consent forms?

There is no e-signature field. Use a checkbox for simple acknowledgements, and handle formal consent through your existing process.

Does it connect to my clinic software or EHR?

There is no direct EHR or practice-management integration. You can export responses as CSV, or send each new response to a webhook or Zapier if your software can receive it.

Can patients get a confirmation email?

Yes, on the Pro plan. Choose the Email question that holds the patient's address and write a confirmation message. The clinic owner gets an email for each response on every plan.

Simple habits that help

Ask for the minimum. A callback request needs a name, phone number and preferred time, not symptoms.
Tell patients why you collect each detail, and link to your privacy notice in the form description.
Password-protect forms that are meant only for registered patients, and close forms you no longer use.
Give most staff the Viewer role, and only share responses with people who need them.
Export and delete responses you no longer need.
